The episode begins with Weaver in the ambulance bay. All around is chaos as police try to secure the area, ambulances wheel away critical patients and press and spectators try to get a good look. Weaver gives some instructions, taking charge, then under her breath gripes that she doesn't even run the ER anymore. She goes back inside which is still a mess of glass and blood. Frank has come in to help. A small boy cowers by the wall and is spotted by EMT Tony Gates. It's Timmy from the previous episode and he's worried about Jerry. Weaver makes her way to the trauma room where Morris and Ray are still working on Jerry and finds him still in critical condition. They're waiting for Abby to bring back a piece of equipment but she hasn't arrived. Weaver goes in search of it herself and finds Abby unconscious and bleeding on the floor in the other trauma room. She calls for help then also sees Luka through the doors to the suture room, still tied to the gurney.
In the van Sam clutches Alex, scared, as co-kidnapper Rafe drives excitedly.
At Michael's graveside Pratt tells Neela that he's been called into the ER. She insists on coming with him.
Weaver extubates Luka who briefly manages to get out what happened before asking after Abby. Weaver tries to make him stay on the monitor but he doesn't listen and goes into the trauma room where a groggy Abby is on a gurney being treated by Morris. The baby is fine on ultrasound. It is a boy. Morris wants to keep Abby down in the ER but she insists on going up to OB. Weaver agrees but insists on Luka, who is wheezing as he breathes, staying down in the ER for a bit.
As Abby is being wheeled to the elevators Neela arrives and asks if she needs help. Abby assures her that she is fine and Neela jokes about which one of them is having the worst day.
Luka goes to help Morris and Ray work on Jerry. He tries to be hands on but is having trouble catching his breath so is relegated to sitting on a stool breathing in terbutaline and observing. He talks Morris through a procedure. Dubenko comes down to take Jerry up to the OR.
In the van Sam tells Steve that Alex's blood sugar is low and they need to get him something to eat. Rafe protests but eventually they stop at a gas station. This also gives them the opportunity to trade in the van for a car. Rafe and Sam go in to get some food. Sam very obviously stuffs her pockets with food under the security camera, trying to get caught, but Rafe spots her and takes the food out. On the way back to the car Sam insists she needs to go to the toilet. While in the cubical she leaves a note asking for help and her identity badge in the hope of someone finding it. Rafe goes into the bathroom after her though and flushes the note. He slams Sam up against a wall and tells her if she tries anything like that again he'll take it out on Alex.
Down in the ER Jerry's mother has arrived. She is combative and complains to Morris about the size of Jerry's pay check and the dangers of the ER. Morris takes her up to the surgical floor before going into the OR observation deck to see how Jerry is doing.
Up in OB Dr. Coburn is assessing Abby's situation. Though Abby can't feel it she's having contractions. The two disagree about treatment methods. Coburn is worried about an abruption but Abby insists she feels fine.
In the OR Dubenko operates on Jerry while Morris and Neela watch from the observation deck. The bullet nicked the aorta. Jerry starts to have a heart attack and his heart has to be shocked. Neela correctly identifies as mistake that has been made in the operation and after this is corrected Jerry's heart resumes a more normal rhythm. Dubenko asks Neela to scrub in to help with the rest of the operation.
The kidnappers' car is stopped at a crossing. An impatient Rafe decides to drive past the queue of cars, between the barriers and over the tracks. They are not hit by a train but a police car travelling the opposite way sees this and follows them. They pull over and Steve and Rafe try to remain calm as the cop asks for ID. When the cop starts to radio back the kidnappers get nervous and take off.
In OB Abby is arguing with Coburn while Luka sits calmly by her side. Cobern wants to prepare the baby's lungs in case they have to do a c-section but Abby has two more months to go and is determined to carry the baby for longer. Coburn doesn't know if she'll have that option. Luka calms an increasingly distressed Abby by telling her it's Coburn’s job to look at the worst possible outcome. Abby gives in and takes the drug.
The car speeds down the country road with the police car chasing it. They dodge a tractor and other traffic. The police car crashes and they get away. Sam tries to convince them to let her go to check on the cop but they aren't listening to her.
In the ER there is still no news of Sam, though they know she was last seen with Steve and the others. Weaver is still running things and has started allowing critical cases back in. Pratt is helping with this. He goes into one of the trauma rooms and finds Frank sitting quietly by himself. Frank gives Pratt a moving speech about how he thought that nothing ever changed but now he thinks that things are just getting worse and the bad guys are winning. Pratt tells him that it was just a bad day for the home team.
Up in OB Abby is feeling better and her and Luka are sitting together calmly. They discuss what to name the baby and Luka makes a joke about naming it Mongo that Abby almost falls for. Her laughter turns to a gasp of pain though and a gush of blood. Luka calls for help and Coburn arrives to confirm that Abby has indeed abrupted and now needs a c-section. As she is being wheeled away Abby begs Luka for them not to knock her out. She wants to be awake.
It's night and the kidnappers have stopped the car at an abandoned truck stop. Steve, Rafe and Mary all argue about what to do next while Sam tries to comfort Alex. The three kidnappers walk out of sight and Sam grabs the chance to rifle through their bags for the car key. Just as Alex is pointing out that Rafe still has it they are both shocked by the sound of gunshots and look in terror in their direction. Steve re-emerges but his two companions do not. Seeing them staring he nervously asks if they want anything to eat.
In an operating theatre Neela and Dubenko work on Jerry while Pratt and Morris watch nervously. Jerry's operation is successful much to everyone's relief.
In another operating room Abby is awake and Luka is holding her hand as Coburn begins the caesarean section. Luka watches the large amount of blood Abby is losing. When the baby comes out it is not breathing. Abby continues to lose a lot of blood. She correctly works out that she's in DIC. The baby is wheeled off to the NICU. Abby tells Luka to go with him. He protests but she tells him she doesn't want their son to be alone. When Luka is gone Abby breaks down. Coburn continues to work on Abby but makes no progress. They can either hope things will get better and risk them getting worse, or perform an hysterectomy.
Alex sleeps in the car and Sam sits outside on the ground, scared. Steve sits down next to her holding a gun and swigging some alcohol. Sam wonders what happened to Steve to make him like this but surmises that the signs where there from the start if she'd just paid attention to them. Steve begins to stroke her hair with his gun-holding hand and tells her than he'd rather they were all dead than parted again. He kisses Sam and pushes her down onto the ground before raping her.
Luka stands in the NICU looking at his son. He is worn out. Weaver comes to check on him and tries to offer some words of comfort but Luka reels off a list of problems that the baby has. Weaver tells him to go and see Abby but he says that she told him not to leave the baby alone. Weaver says he won't be alone.
Jerry is awake in recovery and surrounded by Pratt, Morris and Neela who are all glad he is alright. His mother enters and starts giving him a hard time so the three doctors sneak out.
Luka catches up on Abby in recovery. She tells him that she hopes one baby will be enough and he guesses she had a hysterectomy. He is upset but tells her that one is all they need. She tells him that she'd like to call the baby Joe after her father's favourite boxer Joe Frazier. She is upset about not being able to see him for a while but Luka has taken a video on his phone. The two kiss then Luka excuses himself for a moment. He walks down the hallway and into a bathroom. After vigorously washing his hands he breaks down, sobbing, then grabs a bin and smashes the bathroom mirror with it.
Sam lies on the ground with Steve draped over her in a drunken sleep. Carefully she gets away from him and walks over to the car. She climbs into the drivers seat and is about to drive away when she reconsiders. Quiet and careful she walks back over to Steve and picks up the gun. She pauses for a moment then shoots him three times.
